    This form is used to join the Local Government Super Accumulation Scheme.

    Change of membership details

    This form is used to change your membership details such as name, contact details or date of birth. 

    Remember, for changes that don’t require certified documentation, such as contact details, you can update your details through Member Online.


    Changing investment option(s)

    This form is used to change your existing investment options or to nominate investment options for the first time. 


    Binding death benefit nomination

    This form is used to nominate to whom you wish your benefit paid in the event of your death. Nomination of a beneficiary is optional.

    Member contributions – payroll deduction

    This form is used to have regular super contributions deducted from your pay. It allows your employer to pay your contributions directly from your pay to the LGS account.

    Please review our bank details to ensure you are sending contributions to the correct account.

    Notification of Tax File Number

    Use this form to advise LGS of your Tax File Number (TFN).

    Under Superannuation Law, the Trustee must request that you supply your TFN for superannuation purposes.


    Optional post-tax contribution

    This form is used to make a one-off post-tax superannuation contribution via cheque.

    If you don’t like filling out forms, you can make contributions through BPAY® or EFT. For BPAY® log in to Member Online. For EFT, log in to Member Online or see our banking details.

    Transfer of Contributory Membership

    This form is used to transfer membership to the LGS Retirement Scheme.

    Use this form if you were a contributing member of either the State Authorities Superannuation Scheme (SASS) or Energy Industries Retirement Scheme (EISS); and have transferred in employment to a Local Government employer; and commenced employment with your Local Government employer within three (3) months of terminating employment with your SASS or EISS employer; and have not applied for payment, or rollover, of any part of your SASS or EISS benefits (including the Basic Benefit).
