| Step 1 |
To begin you need to open up the forms section of the site. This page will explain just how this section works. You can access each Scheme simply by clicking on its link in the list at the bottom of the screen. |

| Step 2 |
When you arrive at the Scheme you have chosen you will find a list of the Forms required for that Scheme. Simply click on the name of the form you require to go to that Form. |

| Step 3 |
After you click on your chosen form, you will be presented with some broad information regarding that form. After this passage you may click on a link that will open the form. Alternatively, you may choose to go back by clicking on the 'back' link. |

What is Required for Proof of Identity & Date of Birth?

To be able to process certain transactions (eg request to transfer funds, requests for payment etc),
the law requires that we MUST be reasonably satisfied that we have identified you (ie you are the
person you claim to be) and that we have verified your date of birth.
